Community Solutions' Katie A. program serves youth ages 0-21 that have an open child welfare case or have a behavioral or mental health condition that warrants specialty mental health services. Services are a blend of formal and informal resources provided in the family’s community to help children obtain permanency and stability. A Child Family Team (CFT) approach is utilized to ensure multi-agency collaboration. Services are individualized and tailored to the strengths and needs of each child/youth and their family.
The goals of this program are to reduce out of home placement and psychiatric hospital admissions, increase school achievement, decrease juvenile justice involvement, prevent/decrease child welfare involvement, increase positive family engagement in care, and ensure overall positive experience of care.
Referrals are accepted from the Santa Clara County Department of Behavioral Health Services.
Clients must have Medi-Cal or Healthy Kids and Healthy Families and meet medical health necessity for mental health services.
