Community Solutions’ SOS program serves children and youth ages 0-17.9 who are experiencing at-risk behavior, family conflict, truancy, social/emotional challenges, homelessness, gang activity, relationship conflict, domestic violence and/or behavioral problems.
The goals of this program are to: promote emotional well being and safety within the home, school, and community; diffuse crisis situations and restore family stability; reduce truancy and promote school attendance; prevent or reduce youth involvement in the dependency and/ or juvenile justice systems; provide culturally competent services, and utilize strengths-based models of mental health treatment. Services include:

Referral & linkage to additional services as needed
Aftercare (Full-scope Medi-Cal required)
Referral & linkage to additional services and groups as needed
The SOS program serves Gilroy, Morgan Hill, and the surrounding areas.
This is voluntary program. Referrals are accepted from youth (with parent consent), parents, local schools, the community, and through the Santa Clara County Department of Behavioral Health Services.
